Registration for Thanksgiving Holiday Assistance is completed through a separate registration form. Please visit https://www.metromin.org/holiday-central to register for Thanksgiving assistance.

Photo ID required at check-in and MUST match photo ID on registration.  Guest picking up on bahalf of their family member or friend MIUST show a copy of the photo ID on registration.

Current Utility Bill, Lease Agreement, Rent Receipt. A Driver's License or ID is NOT an accepted form for proof of address.

Upload proof of Need: Food Stamps, Medicare, Medicaid, Disability, Unemployment, WIC, Loss of Income, self-declaration or other document showing need (may be in your child's name)
Preferred Protein
Family will receive a full size from ham or turkey based on availability.

Number of Adults and Children living in the house including head of household.
To ensure accurate registration records, only one parent or guardian may register each child. Duplicate registrations for the same child will be canceled.
